Personal Profile Privacy Notice
Fictional testing only. Real personal and health information collection is closed. This notice describes the current testing arrangements and is not a claim of PHIPA compliance.
Purpose and information
The questionnaire covers participant identity, contacts, interests, relevant health and accessibility needs, orientation and wandering safety, comfort, emergency arrangements and consent. Its intended purpose is to personalize outings and understand support needs. Share only information relevant to participation; a complete medication history is not requested.
Access and protection
Only Alma’s authorized administrator account can submit, read, update and delete fictional profiles. Public visitors, other participants and non-administrator accounts have no access. Answers are kept in restricted Lovable Cloud storage, not in public links or browser storage. Profile information is not sent to outside AI services or used to send automatic emails. Activity records retain the profile reference, time and action, not copies of health answers.
Consent and review
Acknowledgments and their date are saved with the profile. Form submission does not establish a representative’s authority, determine eligibility or confirm an outing. Safety concerns and unclear consent need an individualized review. Family contact must not delay urgent emergency assistance.
Corrections, retention and deletion
Contact Alma to ask about access, corrections or withdrawal. The private review area supports updated answers, relevant changes, a retention review date and individual profile deletion. There is no approved real-client retention schedule yet. Deletion removes the active profile; backup retention and deletion must still be verified with the provider. Minimal activity references remain for accountability.
Before real collection opens
Two-step sign-in, the administrator password, hosting and backup location and protection, provider agreements, a reviewed privacy notice, retention schedule, consent process and breach-response procedures must be verified. Participant authentication and secure invitation arrangements must also be added and tested. Testing acknowledgments do not establish compliance.
